The plan includes workforce redesign, cost and infrastructure efficiencies and long-term sustainability strategies. EPC remains financially well-managed and is not in crisis; this is a proactive, planned shift to ensure future care delivery.

<content><![CDATA[EPC provides holistic, person-centred palliative care to people with life-limiting illness, enabling them to live with the best possible quality of life and, where possible, die in a place of their choosing.

EPC&rsquo;s model includes interdisciplinary support delivered 24/7 in the home, normalising the process of dying while ensuring the right care is delivered, to the right person, at the right time.

While demand for our services continues to grow, funding uplifts have consistently lagged behind the true cost of delivering them. CPI indexation has failed to keep pace with actual expenditure growth, creating a widening gap between funding and the resources required to maintain service quality. This ongoing shortfall places increasing pressure on our organisation&rsquo;s financial sustainability.

We remain committed to supporting our clients and communities, and we will continue to advocate for funding models that reflect the real cost of service delivery. Workforce changes are part of a broader strategy to ensure that we can operate sustainably into the future while continuing to provide high-quality services to those who need them most.

To address this, EPC is initiating a three-year Transformation Plan (2025&ndash;2028) guided by Value Based Health Care principles. The plan includes workforce redesign, cost and infrastructure efficiencies, and long-term sustainability strategies. EPC remains financially well-managed and is not in crisis; this is a proactive, planned shift to ensure future care delivery.

One of the first difficult decisions EPC has had to make is to restructure parts of our organisation. This includes the discontinuation of several allied health therapy roles, specifically in occupational therapy, music therapy, and massage therapy.

These therapies have historically been delivered by qualified practitioners and integrated into our client care to enhance comfort, reduce anxiety, and improve quality of life. They have shown considerable positive impact on the journey of dying and bereaved clients, offering emotional support and therapeutic benefit during some of life&rsquo;s most vulnerable moments.

While deeply valued by clients and families, these services have not been part of our core service agreement and therefore sat outside the funding provided through the Department of Health.

Guided Principles for Decision Making

All decisions regarding service model changes have been guided by the following principles:


	Preservation of Funded Clinical Services: Services that receive direct government funding must be maintained.
	Commitment to Interdisciplinary and Holistic Care: EPC remains dedicated to delivering integrated, person-centred care.
	Future proofing for rising demand: EPC is aware of the predicted tripling of client numbers in the next 10 years, driven by Australia&rsquo;s shifting ageing demographics.


EPC will ensure continued client care through existing services which include those required under the Department of Health&rsquo;s Palliative Care Service Capability Framework. These are as follows:


	Specialist medical professionals
	Specialist nursing professionals
	Psychological, social and spiritual support staff
	Bereavement support
	Volunteer coordination staff and volunteer teams


EPC has offered extensive support to those staff in the discontinued services and staff remaining at EPC.

To help ensure the sustainability of our remaining service offerings, EPC welcomes donations from the public. Every contribution supports our ability to continue delivering high-quality, person-centred palliative care to those who need it most.

<content><![CDATA[A heartfelt thank you to Freemason&rsquo;s Foundation Victoria and Blackburn Freemason&#39;s Lodge who have generously donated $10,000 and $2,600 respectively, which is a total of $12,600 towards EPC&rsquo;s Duress Alarm program. 

For our clinical staff who go out every day to people&rsquo;s homes, especially overnight, having a small discrete Duress Alarm on their person means that at the press of a button, someone is there to help.  This means our staff can feel safe and confident to concentrate on their important work of helping terminally ill people in our community to live well every day.  

We are so honoured to be the recipients of this generous donation. Thank you to Dr Andrew Barnden AALIA GAICD for facilitating this.

<content><![CDATA[As part of a new initiative, Eastern Palliative Care (EPC) partnered with Palliative Care Victoria (PCV) to offer a free scholarship for a &ldquo;Train the Trainer &ndash; Biography Gold Level&quot; which was awarded to Volunteer Coordinator, Alison Woolley of Anam Cara Colac.

The scholarship allowed Alison to complete the &quot;Train the Trainer&quot; program in biography writing, equipping her with the skills to lead the new biography writing service at Anam Cara Colac. Alison said &ldquo;The training was thorough, thoughtful and supportive, covering a wide range of scenarios and strategy to support the program,&quot;. <content><![CDATA[A wonderful New Year Celebration was held at our Mitcham office recently. Over 100 staff and volunteers enjoyed getting together in a relaxed environment to celebrate the New Year as well as EPC&#39;s 25 year anniversary.


We discussed exciting developments such as the roll out of the new EPC Foundations course which equips new palliative care nursing staff and allied health professionals with the skills and confidence to deliver safe and proficient community palliative care.


We also recognised Committee Of Management member and Public Relations and Fundraising Subcommittee Chair, Margaret O&rsquo;Donnell, and former volunteer, Lyn Read, who were both honoured with an OAM in the Australia Day Awards. Margaret was awarded for her service to education and community whilst Lyn was awarded for her service to community health including her 17 years as an EPC volunteer.

We&#39;d like to acknowledge and celebrate our fantastic team of EPC Volunteers. Their work covers a multitude of areas ranging from dog walking, photography, bereavement, respite and administration through to biography work, phone support, hair dressing and working directly with our carers both pre and post death.

We are so grateful for their kindess and hard work, for helping the community and improving quality of life for our clients and for enriching lives with every day acts of kindness. Their visit changes the day.

<description><![CDATA[EPC were honoured to recently receive a donation from the Lodge of Quest in Ringwood. Two of their members, Steve and Wayne, attended our offices to meet with Gaylene, our CEO, to hear more about EPC services and what equipment we can provide to our clients to help keep them at home and avoid going into hospital or a hospice.

<content><![CDATA[EPC were honoured to recently receive a donation from the Lodge of Quest in Ringwood. Two of their members, Steve and Wayne, attended our offices to meet with Gaylene, our CEO, to hear more about EPC services and what equipment we can provide to our clients to help keep them at home and avoid going into hospital or a hospice.

Steve and Wayne then presented a cheque to Gaylene to the value of $6,500; comprising funds from the Lodge themselves and a grant from the Freemasons Foundation. We will use this donation to purchase a portable hospital bed for our clients to use.

We look forward to a continuing relationship with the Lodge of Quest and with the Freemasons Foundation and thank them both for their generosity to date.

What is your role at EPC?

My role at EPC is as a Coordinator of Volunteers. My role incorporates recruitment, training, coaching and leadership to a team of volunteers.


Why did you pursue this role/career?  

After having my first child (she&rsquo;s now 11), I decided that corporate life wasn&rsquo;t for me &ndash; it was a incredibly demanding job with a lot of travel and long commutes and I wanted to do something that included giving back to the community. Part time really appealed so that I could have some work/life balance and spend time with my child. Nearly everyone at EPC works part time so it is a good balance.  

How long have you worked for EPC? 

10 years in October

Has your career progressed since you&rsquo;ve started?

It sure has. After 6 years in a learning and development focussed role, I was looking for my next challenge and a role came up within the Volunteer Services Division. 
My background had been in a number of specialist HR roles and this just consolidated all those skills as well as broadened my leadership background.  


What stands out to you about EPC as an organisation?

I think if you are someone that wants to give back there is incredible sense of satisfaction and fulfilment in working at a place like EPC. It&rsquo;s a real privilege being part of this space. Our volunteers often say they get so much out of this work than they give.

We have some exciting initiatives presently around diversity and inclusion and First Nations Peoples  and staff can get involved in these working groups to suggest policy and procedure changes.

EPC support further training and development and encourage staff to build on their skills.
Training is regularly provided and supported externally.

What is something about you that others may not know?

I studied a zoology degree but ended up working with humans. I do also have a psychology major so I&rsquo;m not completely underqualified to be in this space.


What is the best part of your job and why? 

The best part of my job is working with over 200 incredible volunteers, we are a diverse bunch and we have a lot of fun. We also do some really special work for our clients, that feeling that you&rsquo;ve turned someone&rsquo;s day around when they&rsquo;re going through so much is pretty incredible. So, for anyone watching I&rsquo;d encourage you to jump on our website and have a look. There are so many ways to help out someone who is dying and some of them you may not have thought of.

I think I felt before I understood palliative care that it would be incredibly sad, all the time. Whilst of course there is sadness, predominately we are helping people to live well while they are dying. 
There&rsquo;s something about a volunteer visit that can take our clients away from the medical focus and that can be freeing. 

I also adore my crazy team, we work really hard but there is always time for hilarity. There is a lot of joy around our work, both in the office and out with our clients. 


What is the most challenging part of your job? And how do you deal/overcome this? 

I think in vol services it&rsquo;s just the pure volume of information we are taking in regarding our volunteers, clients and keeping everyone informed. We have a great manager and a great team and really great processes at EPC so it works really well.

Describe a typical day for you?

I don&rsquo;t think there is such a thing as a typical day in vol services.
We never know what&rsquo;s going to transpire&hellip; It could be a client or vol issue we need to help solve, or a vol may drop in or call and need to debrief, we get referrals from the wider EPC staff and we work matching clients with the right volunteer. We plan and run fun and educational events for our volunteers. We have a recruitment season and run that completely and we train a new intake of vols every year. 

Who inspires you? This can be a colleague, author, celebrity, world leader etc

My kids inspire me&hellip; I&rsquo;ve learnt so much about myself watching and helping them grow.

What is your role at EPC?

As a nurse my typical day involves being on the road and seeing clients to help them manage their symptoms optimally. These visits may be straight forward, involve some tough conversations, or caring for someone who is actively dying in their home. Each day is different and presents unique opportunities for learning and client engagement.

Why did you pursue this career?  

Nursing is my second career. I previously worked for a pharmaceutical company for 15 years, burnt out and randomly resigned one day. When I left, I started telling people I was going to pursue nursing, more for something to tell people than for any real desire. My mother found a post graduate &lsquo;entry to nursing&rsquo; course I could do and off I went, because &lsquo;why not&rsquo;. As part of my training, I had the opportunity to do a community placement with Melbourne City Mission and found my calling by accident rather than design. 

As part of my nursing graduate year with Peninsula Health, I worked on their palliative ward for 4 months, and my interest was consolidated. At the end of my graduate year, I applied to palliative wards around Melbourne. EPC were crazy/bold enough to hire a new nurse straight out of her graduate year, and I have loved working for them ever since.

How long have you worked for EPC? 

Just over three years, I started the month of the pandemic in 2020.

How has your career progressed since you&rsquo;ve started? 

Since starting with EPC I have progressed through the standard training of a Grade 3 EPC nurse and have had the opportunity to be acting CNC when my manager is away. My previous workplace experience as a manager has helped me in this advanced role and I have enjoyed the wider organisational understanding it gives me. 

What is something that stands out to you about EPC as an organisation?

The fantastic support I felt as a nurse during the pandemic. It was important that we continued to provide care to our clients but it was also really important that the nurses were protected and supported to do their role effectively. 


What is something about you that others may not know?

I am a huge introvert. I have developed strategies to enable me to work/play with others well, but I need time out to concentrate on self-regeneration/care on my days off. Without this I can turn into a pile of quivering insanity with poor emotional regulation. 

What is the best part of your job and why? 

After my previous job where I worked at least 50 hrs a week and did a lot of travel, the thing I really value about my job is that after a hard day&rsquo;s work I can walk away and the next day is a brand-new day. If I have done my job well, everything is taken care for my clients that day, either by my actions, or by follow up actions I have scheduled. While I am not terribly good at disconnecting from work, I do like that I don&rsquo;t have to hold things over till the next day if I have done my job well.

What is the most challenging part of your job? And how do you overcome this? 

I am able to deal with the anxiety and stress I find in client&rsquo;s homes, but find dealing with colleague&rsquo;s excitement/anxiety/stress challenging. I find it harder to distance myself from my colleagues&rsquo; concerns. I think this probably comes back to my introversion. 

How do I overcome this? Just breathe and listen.

What is your role at EPC? 

My role as Clinical Nurse Consultant is to provide clinical leadership to the nurses in the aged and disability support team, the intake team and the nurses who work the evening shift.


Why did you pursue this role/career?  

I completed my first ever nursing placement 3 weeks into my degree at Bethlehem Hospital in the hospice.  It was confronting as an 18 year old, but I recall the compassion and calm nature of the nursing staff.  They were so skilled at having difficult conversations.  I really was in awe and felt it was something I would love to do, but I felt that I needed both clinical experience and maturity to become a palliative care nurse.  
When I did start working with EPC, I was regretful that it had taken me so long to get here!

How long have you worked for EPC? 

I have worked with EPC over the last 5 years across multiple teams.  

How has your career progressed since you&rsquo;ve started?

I started with the inner team as a clinical nurse then moved to the central team to be closer to home. 
Then in December 2022 an opportunity arose to move up into a leadership role and with Senior Management&rsquo;s support I started as Clinical Nurse Consultant in the region wide services team.  I have always been drawn to leadership roles, and was motivated to work to support the nursing team at EPC to provide optimal care to clients as well as to maintain their own well-being.  

What is something that stands out to you about EPC as an organisation?

EPC is incredibly passionate about keeping client care at the centre of everything we do. I have never worked in an organisation who has been so responsive to client needs. 
EPC also invests in its staff.  I was blown away that teams are allocated multiple days per year to come together, reflect on the clients they have cared for and take some time to connect with one another.  It is important to recognise the stress that can come from caring for others, and EPC continues to develop ways to support our staff. 


What is something about you that others may not know?

I&rsquo;m a big sharer, so most people know most things about me! We have a new addition to our home.  My 12 year old son recently purchased himself a snake. So now we have three cats, a 60kg dog and a snake. It&rsquo;s quite a farm.  


What is the best part of your job and why? 

I really love the clinical work. The complexity of the symptoms experienced by our clients can be very overwhelming and it is satisfying when we are able to improve quality of life with our interventions.  
I also really enjoy motivating and developing the nursing staff.  We have nurses with an exceptional amount of skill and knowledge. Finding ways that they can confidently work to their maximal scope of practice is the best part of my leadership role.  


What is the most challenging part of your job? And how do you deal/overcome this? 

Self-care is not always easy as a busy working mum. It is hard to remember to structure time for the things I need to prevent burnout. I have learnt to prioritise this (most of the time!) in order to provide an example to other nurses, as well as my kids.  


Describe a typical day for you?

The start of the day is busy. Our team prioritises visits for the aged and disability care teams, reviews new referrals and follows up any urgent matters regarding clients waiting to be admitted to EPC. The middle of the day involves meetings, project work and time with my team discussing process improvements. The afternoon is spent organising work for the evening nurses, debriefing and assisting the intake team when needed.  


Who inspires you? 

I am inspired by my parents.  They started their life together with very little, and have worked incredibly hard and have made sacrifices to build successful careers where they are both highly valued, as well as a big, crazy and loving family.  They have taught me the importance of prioritising family and working with purpose.

<content><![CDATA[Drey has been with EPC for 20 years!  Learn more about Drey and the life of a PalCare (Patient Records) Administrator.

What is your role at EPC? 

I&rsquo;m the PalCare Administrator my day is full and varied with no day ever presenting as the same.
PalCare is a client management system that houses our client medical records and allows our staff to access client records on the road and in real time.

I manage the day to day operations, provide data analysis and reporting for clinical staff, senior management, the board and department of health.  I also spend a considerable amount of time working on development of the product and planning how we can improve PalCare to ensure our staff are able to do their work in the most efficient way.


Why did you pursue this role/career?  

To be honest I fell into this role having started as an Admin Assistant with the Central team after starting a family 20 years ago.

Our then Business Manager invited me to a working party meeting focused on moving our medical records to an electronic platform. Post that meeting a role was created and I was asked to fill it.  19 years on and I&#39;m still here and loving the day to day challenge the role presents.

What is something that stands out to you about EPC as an organisation?

I consider EPC an organisation full of wonderful humans who all work hard for the collective good of others.  Everyone contributes in their way to make a difference and that feeling is uplifting and empowering.

What is something about you that others may not know?

I am involved in a number of community organisations:

School Council at my daughters school
Basketball Coach &ndash; domestic and representative
Surf Life Saving Nippers Coach
Footy Club Team Manager

What is the best part of your job and why? 

Diversity.  No day is ever the same, the are constant challenges and that keeps things exciting.

What is the most challenging part of your job? And how do you deal/overcome this? 

There are parts of my job that rely on others.  It can be really frustrating to have to problem solve and manage issues that have presented because of someone elses actions. I deal with this by communicating the system requirements with staff and managers.

Describe a typical day for you?

4.15am wake up call
5.00am Gym class
Sort kids out
Head to work
Data download
Review and respond to emails
Review demands and priorities and re adjust depending on what is new
Respond to calls for help and support
Run data analysis, crunch data, balance figures
Communicate with PalCare
Create queries
Send reports
Leave work

Coach, attend committee meetings, etc.

What does your role involve?

I visit clients in their homes and provide symptom management advice and practical support right from when they are first referred and assessed through until end of life. This can look like medical advice, emotional support and hands on end-of-life care including subcutaneous infusions. 

Why did you pursue this role/career?  

Palliative care has been an interest of mine since I started nursing. I have found that caring for people with terminal conditions is the most rewarding for me. We can develop such a strong rapport with both clients and their families at this special time. 

What is something that stands out to you about EPC as an organisation?

EPC are so well known and our reputation is excellent. We have an extensive skill mix and specialist medical support readily available. I feel that the nursing cohort is also highly valued by senior management which is incredibly important to me in this field of work.  

What is something about you that others may not know?

I originally planned to go into the Police Force!  

What is the best part of your job and why? 

My favourite part of my job is when I&rsquo;m rostered onto the office duty nurse role. I really enjoy leading the team and managing all incoming client issues/crises. You never know what is going to come in on the end of the phone.

What is the most challenging part of your job? And how do you deal/overcome this? 

The most challenging part of my job is looking after young children. Symptom management, emotions and family dynamics are often far more complex for these clients. Having young children myself definitely adds another layer to the challenge. I make sure that I debrief regularly and that as a team we share the care of these very special clients. 

Describe a typical day for you?

I arrive at work 30 minutes before my shift starts and the Inner team share breakfast together &ndash; we make sure to laugh a lot and really relax before we face the day. I then visit my clients in their homes &ndash; some are stable and not needing anything changed, others are more complex and need new medications or have entered the terminal phase (or end of life care) which then triggers another set of tasks that we complete in order to ensure that client and family are well supported.  I then return to the office to complete any notes/assessments/correspondence and hand over my findings to our clinical nurse consultant. 

Who inspires you? 

My inspiration is my dad. He raised my brother and I as a single parent yet still felt fulfilled and is happy. He supported me through 2 university degrees and the demanding nursing placements. 
I hope that I can be as successful and determined as he was, plus also a role model for my three daughters.

1 October | Commenced the provision of home-based palliative care services across the whole of the Eastern Metropolitan Region, becoming the largest single agency provider of palliative care services in Victoria.


1999 - 


	Code of Ethics adopted. Vision, Mission &amp; Values developed.
	New logo developed.
	


2001 - Accreditation received from the Australian Council of HealthCare Standards

2004 - Development of PalCare - our Client Information Management System - supported by a seeding grant from The Ian Potter Foundation.
    

2005 - State Winner of the 2005 National Australia Bank Volunteer Awards
2006 &ndash; Intake team introduced
2008 - 10 years of service in the local government areas of Boroondara, Knox, Manningham, Maroondah, Monash, Whitehorse &amp; Yarra Ranges

  &bull;    EPC works closely with a migrant resource centre to translate information brochures into a variety of languages.

<content><![CDATA[Precisely 1511 Biographies have been produced as at the end of November, an incredible feat of our Volunteer EPC Biographers. 

Although other organisations run the same or similar program, EPC has achieved the highest number of biographies in a community based palliative care organisation.

96 of EPC&rsquo;s 220 volunteers have received additional in-house training to become palliative care biographers. Two products can be produced. One is an adult memoir with photos. The other is a memoir in the style of a children&rsquo;s picture book for dying parents to create and leave for their children. 

Over the years, we have been privileged to hear stories from prisoners of war, refugees, prostitutes, business people, housewives, hobbyists, politicians, and journalists. People close to 100 years old and people as young as 16. We have listened to stories of mental health, family secrets, disability, suburbia, survival, stellar success and desperate disappointments. We have heard funny stories and sad stories. Each and every story has been intriguing. Each and every story has been valuable.

The process of storytelling enables messages, philosophies, beliefs, memories and culture to pass from one person to the next.  Many clients also report a reduction in anxiety, depression, pain and breathlessness and an increase in self esteem as a result of undertaking this.

Lee, one of our Biographers, says of the process: &ldquo;[The clients] Pain seems to be completely out of their thoughts while we&rsquo;re doing it. While they&rsquo;re reminiscing they&rsquo;re in a different world of memories..&rdquo;   

The finished pieces are appreciated by clients and their families, we received this testimonial earlier this year:

&quot;On Thursday last, Stella Hewson delivered to me the final edition of my biography which I started at 50 years of age and due to my current ill health, would never have finished. 
What a great day it was when she first visited me to discuss the project (I never knew that service existed). Such a kind capable and understanding person. 
Over several visits to my home and discussions, she succeeded in turning my rough draft into something far more professional which gives me comfort, closure, and fulfilment. It is another part of my plan and preparation towards acceptance and knowing my children in particular, may understand me. In the words of the song &quot;say it loud, say it clear&quot;.  I am in deep gratitude to Stella devoting so much time and thought into making this happen and to Eastern Palliative Care for making it possible. &quot;

The EPC Biography program has received lots of wonderful recognition in its time such as the Minister for Health Volunteering Award in the category of &quot;Outstanding Achievement by a Volunteer: Improving the Patient Experience Award 2018&quot;, features on television programmes ABC&#39;s 7.30 Report, Channel 10&#39;s The Project, multiple programs on Radio 774 (State and National) and various Newspapers. 

  

There is even a research project being undertaken titled Storytelling in Life and Death by Karly Edgar in conjunction with La Trobe University. 

EPC also trains other not-for-profit palliative care organisations to run their own volunteer based palliative care biography program and EPC&rsquo;s model is now replicated all over Australia.

We&rsquo;re revolutionizing the Biography program in 2022 running a pilot project where bereaved carers are able to work with biographers and have their story recorded too. We have called this part of the program &ndash; Chapter 2.

Here is some feedback from 2 carers and the biographers who have done the work with them.

&ldquo;My bereaved client was very impressed with the final product. He thanked me and EPC for the time we had given him on this project. He told me it helped to distract him from his loss for a while.&rdquo;  Biographer &ndash; John Addison

There&rsquo;s lots happening in our Volunteer Biography area but what remains the same is our focus on clients, carers and families and creating memories to treasure into the future.

Gaylene stated &ldquo;after being a community palliative care specialist Registered Nurse and Nurse Educator for several years, I feel like I am coming full circle in accepting this role. I am very privileged to join my passion for the provision of high quality specialist palliative care in the setting of a client&rsquo;s choice with the values-based leadership and management expertise I have learnt over the intervening years.&rdquo;

Gaylene is very much looking forward to joining EPC, bringing with her a wealth of knowledge, enthusiasm and experience. Gaylene starts with us on Monday 1 August.
